Acute myeloid leukemia with myelodysplasia-related changes (acute monocytic subtype) with interstitial del13q, ASXL1 mutation (c.1900_p.Glu635fs), RUNX1 mutation (c.805 + 2 T > A), and two NRAS mutations (NRAS c.182A > G, p.Gln61Arg and NRAS c.35G > A, p.Gly12Asp). A Blood smear with a myeloid blast, monoblast, several promonocytes/abnormal monocytes, and occasional dysplastic neutrophils. Higher magnification of blood smear demonstrating B a myeloid blast and promonocytes/abnormal monocytes, C dysplastic neutrophil–hyposegmented and hypogranular (black arrow), and an abnormally large and hypogranular platelet (red arrow), and D erythrophagocytosis by a leukemic blast following chemotherapy
